Consent and registration forms are required for students to receive services in the Brooks Health Center. Please complete and sign each registration form and return to the school prior to calling in to schedule an appointment. These forms can be given to the Health Center staff, the School Nurse, or emailed to BrooksHealthCenter@vvsd.org.

Who can use the health center?
All VVSD students may utilize the Health Center. 

Does my child need my permission to use the health center?
Students are required to have their parent or guardian complete and sign the consent and registration forms prior to receiving services at the Health Center. 

What does it cost to use the Health Center?
Services will be billed to your insurance (All Kids, Medicaid, private insurance) at the time of service. Families are encouraged to contact the Health Center if they do not currently have health insurance and/or are unable to pay for medical services. We will assist with the All-Kids enrollment process or explore other options you may qualify for.

My child has private insurance, what will she/he have to pay to use the health center?
At the time of scheduling, your appointment, health insurance information will be collected and verified. The parent/guardian will not be billed for any co-pay required by his/her private insurance.

My child does not have health insurance. Can she/he still use the health center?
The Health Center aims to provide care to all students regardless of ability to pay. Health Center staff will work with you to make arrangements so your child can benefit from the services provided by the Health Center. Families without insurance will be asked to meet with a Health Insurance Enrollment Counselor who can assist with applying for Medicaid or Marketplace health insurance.

How will the Health Center work to protect my privacy?
All information related to your care is confidential and will be kept private. Your medical records will not be shared without your consent unless required by law.

Medicaid provides affordable health coverage to residents of Illinois.  The term “Medicaid” is used in this guide to describe the health coverage programs managed by the State of Illinois.  Medicaid is sometimes referred to as “All Kids” or the “medical card.”
